DOCTORS APPOINTMENTS

Please send all medical queries and Doctor appointment requests via our website  The Selsdon Park Medical Practice and click on the Doctors appointments section which will take you directly to the form to be completed

You can access all online request forms either through the website or on your NHS App. We will respond the same day, allowing those with urgent needs to get prompt appointments.

If you do not have a smartphone, please call the practice. Our Reception Team are here to help guide you through the simple questions and help you complete the online form if you are unable to complete the form online for medical or accesability issues.  All requests will be assessed as soon as possible and in the same way, regardless of how you contacted the surgery. https://accurx.nhs.uk/patient-initiated/h83018 

If an appointment is needed, you will be contacted by phone or sent an SMS text message with a self-book link.  If the GP feels you should be seen today you will be contacted by phone initially.   Please ensure that you are able to take the call from the surgery if submitting an online triage form. 

The aim is to make access easier for everyone.  This new system allows those who need to be seen urgently are seen.
